The signs and symptoms of multiple sclerosis (MS) depend on three main ways the disease can manifest:

1. Relapsing Remitting Disease

The common initial presentation is weakness, numbness, tingling or unsteadiness in a limb, spastic weakness of legs, constant aches and pain, blindness, double vision, difficulty to maintain the balance of body or a sphincter disturbance such as urinary urgency or hesitancy. Symptoms may disappear after a few days or weeks.

In most patients, there is an interval of months or years after the initial episode before new symptoms develop or the original ones relapse.

2. Secondary Progressive Disease

In this presentation of multiple sclerosis, relapses and usually incomplete remissions lead to increasing disability, with weakness, spasticity, and imbalance of the limbs, impaired vision, and urinary incontinence. In some of these patients, the clinical course of multiple sclerosis changes so that a steady deterioration occurs, unrelated to acute relapses.

3. Primary Progressive Disease

In this manifestation of multiple sclerosis, symptoms are steadily progressive from their onset, and disability develops at a relatively early stage. This course of disease is less commonly seen.

Many factors like infection or trauma can trigger exacerbations of multiple sclerosis. Relapses may occur even after two to three months of pregnancy possibly because of post delivery stress.

MRI scan of the brain and cervical (spinal) cord is quite helpful in making diagnosis.

The available medical treatment is not effective and usually frustrating. Mostly steroids are used which have their own side effects. Moreover long-term treatment with steroids provides no benefit and does not prevent further relapses.

The evidence of benefit of immunosuppressive therapy is also incomplete. These drugs also have deleterious side effects.
